The aim of this study is to determine the association between the midline diastema and the labial frenum, as well as the best treatment in each case. Material and method. An electronic and manual research has been carried out excluding those articles regarding syndrome patients, or those that are only related to surgical techniques. Results and discussion. 40 articles were selected and 6 books were consulted. The etiology of the diastema seems to be multifactorial, being a hypertrophic frenulum the main cause in some of the cases. Although there are different strategies amongst the literature to deal with this phenomenon, articles with the most scientific evidence, following physiopathological reasoning, recommend starting with the frenectomy followed by immediately closing the gap with orthodontic treatment. Conclusions. Nowadays, there is no protocol with enough scientific evidence about when to intercept and how to treat the midline diastema. Its treatment has depended on expert opinions over the last years.
