This is a clinical case of an asymmetric extraction patient with facial and dental asymmetry, deviation of the upper midline, inclined occlusal plane, a narrow palatal and chin deviation to the right. The patient refused the surgery option and was treated with the extraction of the 1.4, dental expansion, lower anterior stripping and dental compensations of the asymmetry were leaving. We solved the smile esthetics, claim of the patient.  (Rev Esp Ortod. 2013;43(2):097-106)
