Starting a clinical treatment case with uncertainty, with traditional approaches can be hazardous, inefficient or even dangerous for the growing risky patient. Therapy guided with the discrimination of the growing risky patient, with the framework of growth anomalies and long range growth forecasting equips the clinician with such knowledge; starting with the end in mind. The long range growth VTO renders not only the timing but the planning of the treatment more accurate. The statistical reliability and the utility of a forecast cannot be judged by linear or angular dimension correlation but by clinical decision making, by clinical outcome. A long term clinical example is shown to illustrate how to say NOW or LATER. The new millennium practice will be based upon preventive medicine with a philosophy of anticipation in order to reach excellence.  (Rev Esp Ortod. 2002;32(2):159-175)
