Report a case of adult patient with Class III and open bite. We performed the treatment of the case by combining orthognathic surgery and orthodontics. In the first step brackets were placed to align and we simulated a surgery in models. Later orthognathic surgery was performed in advance and maxillary impaction, asymmetrical mandibular set-back and mentoplasty. Finally placement of the implant and set the occlusion.  (Rev Esp Ortod. 2008;38(3):223-236)
