Time: the fourth dimension in treatment planning for Class II malocclusion

Aim. To demonstrate the role of treatment timing on the outcomes of dentofacial orthopedics for treatment of Class II dentoskeletal disharmony. Materials and methods. A controlled clinical trial on the results of Twin Block therapy of Class II patients and a very recent meta-analysis of the studies on functional jaw orthopedics revealed the significant role of timing of treatment on treatment modifications. Results and conclusions. The results of methodologically-sound cephalometric studies of both the past and the recent history of orthodontics clearly indicate that optimal treatment timing for Class II skeletal disharmony with a functional appliance (e.g. Twin Block) is during or slightly after the peak in mandibular growth as revealed by a reliable biologic indicator of individual skeletal maturity such as the cervical vertebral maturation method.  (Rev Esp Ortod. 2011;41(4):199-204)
